VBS July 8-12, 2012 (9 am - 12 pm each day)

 

Registration (Ages 3 YRS - 4th Grade)

 

Registration Fee $30 (max $60 for families, siblings only), due at registration. Registration can be completed two ways: online (complete the form below) or by submitting forms to the HopePointe Church Office either in person or by mailing them. For registration to be complete the registration form page and one liability page per child must be returned to HopePointe Church Office by July 5th, 2013.  Liability must have an original signature prior to the start of VBS.

 

If you are chosing to register online, there is no need to download these forms.

Forms of payment accepted are online through PayPal (part of the form below), cash, or check made out to HopePointe Anglican Church.

 

Limited space available, completed paperwork and payment secures your spot.
